If you suspect one of your learners has a visual impairment please download the relevant form below. Anyone can refer a child to our service but you must have the consent of the parents/carers.

If you are referring your child to our VI service, they must have visited their optician in the last 12 months.

Always contact your GP if you suspect a change in your child's health or development.

Windsor Park are no longer taking referrals for hearing tests. This must now be checked by your GP.
